	Fee Schedule Daily Referral Fee Schedule For Temporary Services: In addition to paying the agreed upon pay rate directly to the referral candidate, the client will pay HB STAFFING DENTAL PLACEMENTS a daily agency fee for each day referred candidate works pursuant to the following fee schedule:


	• Front Office Staff • Dental Assistant • Dental Technician • Dental Hygienist • Office Manager • Dentist


	$25.00 per day $35.00 per day $35.00 per day $40.00 per day $40.00 per day $55.00 per day

Payroll Option: Some clients prefer that HB STAFFING handle the payroll of the Temporary employee.

In that case, we charge you a 65% markup on the hourly rate that we pay the temporary candidate. In this case, we will handle the payroll, tax deductions, workers compensation and administrative costs of employing the temporary.
In order to exercise our Payroll Service Option, we will need a written agreement to be signed by client/us prior to start of assignment and credit application/credit card on file to guarantee payment.

Direct Hire Fees:

Dentist: $4,950.
Office Manager OR Dental Hygienist: $3,600.
Dental Assistant or Dental Technician: $3,150.
Front Office Staff: $2,250
